Disclosures: Simpli Dental is an independent distributor contracted to sell discount dental plans for USA Healthcare Savings/Capella Group which are affiliate companies of Access Plans, Inc. This discount dental plan is NOT insurance. This plan provides discounts for certain dental services. Discounts for vision, prescription drugs, hearing, chiropractic/ alternative medicine are also included. The range of discounts for dental services offered will vary depending on the type of provider and products or services received. The Plan does not make and is prohibited from making members payments to providers for products or services received under The Plan. The participating member is required and obligated to pay for all discounted services including dental, prescription drugs, vision, hearing, and chiropractic. All Members subscribing to the USA Healthcare Savings discount dental program (“Program”) should read the terms of this Agreement carefully and communicate any questions that arise to a Capella Member Services Representative available by telephone Monday – Thursday, 7 a.m. to 6 p.m. and Friday, 7 a.m. to 3 p.m., Central Time at 1-888-411-3888. You have the right to cancel your membership at anytime. If you cancel your membership within 30 days from receipt of your membership card and materials, you will receive a full refund of your membership fees*. Cancellation requests must be in writing. We will stop collecting membership fees in a reasonable amount of time, but no later than 30 days after cancellation. IF A MEMBER CANCELS WITHIN THIRTY (30) DAYS OF THE EFFECTIVE DATE, USA HEALTHCARE SAVINGS WILL REFUND THE INITIAL MEMBERSHIP FEE. THE ENROLLMENT OR APPLICATION FEE IS NON-REFUNDABLE (EXCEPT WHERE PROHIBITED BY LAW).
